CONSENT FORM AND CONTRACT

I, hereby agree to the following terms and conditions regarding the care of my dog, [Dog's Name], while in the care of Harry's Home Boarding:

1. Veterinary Care: In the event of suspected injury or illness to my dog, I authorize Harry's Home Boarding to contact a veterinary surgeon (vet) to examine my dog and perform any necessary investigations or treatments, as advised by the vet.

2. Usual Vet: I understand that, where possible, any treatments will be undertaken by my dog's usual vet. However, if that is not possible, the treatment may be provided by the nominated vet.

3. Prescribed Treatments: I agree to the premises administering any prescribed treatment that the vet considers advisable.

4. Expenses: I understand that all consultation, testing, and treatment costs will be at my own expense.

5. Euthanasia: I give consent for euthanasia if it is recommended on humane grounds by the vet caring for my dog.

6. Communication: I understand that every effort will be made to contact me or my emergency contact to discuss the appropriate course of action and provide updates throughout the process.

7. Fleas and Worms: I understand that if my dog has fleas or worms, the premises will take the dog to the vet to arrange appropriate treatment, and I will be responsible for the vet's bill.

8. Feeding: I consent to my dog being fed at the same time as dogs from other households, with the understanding that efforts will be made to separate dogs while feeding.

9. Walking: I consent to my dog being walked outside of the premises or garden, and within a group of dogs from other households (not exceeding 4 dogs).

10. Cohabitation: I consent to my dog being kept together in the house with other dogs.

11. Health Checks: I consent to my dog receiving a health check while in the care of the facility, which will include checking the teeth, ears, nails, and for the presence of parasites such as ticks or fleas.

I hereby confirm that the above details are correct, and I accept full responsibility for my dog while it is in the care of Harry's Home Boarding. I acknowledge that I will bear all costs incurred due to any injury, damage, or other incidents involving my dog, at my own expense.

WHAT TO BRING This is entirely up to you and your dog. We encourage owners to bring items such as beds, blankets, leads, and harnesses. While we do provide all of these items, bringing your own helps ensure the best home-from-home experience for your dog. You will also need to bring your dog's food. We have plenty of treats available, all of which are low fat and grain-free, and these are complimentary. If your dog is on a raw food diet, please be sure to inform us in advance.
